Documentation Guidelines

Purpose of Documentation

At BSC, we are dedicated to assisting you in navigating the documentation processes necessary to ensure that you receive the accommodations you need for a successful and accessible educational experience. Documentation is essential to:
  • Establish disability or health condition
  • Understand how the disability or health condition may impact the student
  • Guidance for accommodation decision-making

Documentation Formats

  • Professional letter from a qualified provider on their letterhead
    1. A Few Examples of Qualified Providers:
      1. Medical Provider
      2. Veterans Affairs
      3. Psychiatrist
      4. Psychologist
      5. Vocational Rehabilitation
      6. Etc.
  • Individual Education Plan (IEP) or 504 Plan with testing information
  • Testing Evaluations (Learning Disability, ADHD, etc.)
*This is not an exhaustive list; please reach out to bsc.accessibility@bismarckstate.edu if you have questions about submitting an alternative form of documentation.
 

Helpful Information for Providers to Include

  • A diagnosis of the disability or health condition and its severity
  • Date of diagnosis
  • Date of the most recent visit with the medical provider
  • A description of the limitations or impacts the disability or health condition has on the student and what barriers it may create
  • Information explaining if the disability or health condition is permanent or temporary
    1. If temporary, please provide information on the expected duration and timeline for re-evaluation of the condition.
  • Identification from a qualified healthcare provider who submitted the documentation including:
    1. Contact information
    2. Credentialing and licensing information
    3. Their Signature

How to Submit Documentation

  • Students may upload documentation through the application portal after submitting their Student Accessibility Application.
  • Students or their medical professional/provider may send the student’s documentation via email, mail, or by dropping it off at BSC’s Student Accessibility Office.
